Where to find your weight from when GLP-1 was first prescribed

We need documentation showing your weight from when your GLP-1 medication was first prescribed. Please ensure your name, date of birth, and weight are clearly visible.

Where to find this information:

Medical visit summary

After your appointment, your doctor likely provided a visit summary or after-visit summary (AVS) that includes your vital signs, including weight.

Patient portal records


Log into your healthcare provider's patient portal (MyChart, FollowMyHealth, etc.) and look for:

  • Visit notes from your initial GLP-1 appointment
  • Vitals or measurements section
  • Clinical notes or encounter summaries

Prescription or prior authorization documents


Some GLP-1 prescriptions require insurance pre-authorization, which often includes your weight and BMI. Check any paperwork you received with your initial prescription.

Lab results or clinical documents


Any medical document from that appointment date that includes vital signs, such as lab orders or test requisitions.

Doctor's office

If you can't locate this documentation, contact your prescribing provider's office and request a copy of your visit summary or vitals from your initial GLP-1 appointment.
